Incident Report
Create a factual record of an incident and the actions taken. One entry per incident keeps the time, location, description, and follow-up context together.
Make it your own
On the web, sign in and choose New Dattle → Template. Web Dattles use Shared access; you can start with just yourself and invite people later. For a Private Dattle stored on your device, use the mobile app.
Start from the template
In New Dattle, choose Template and search for Incident Report, or browse Field collection. Open its preview, choose Use template, and review the name and fields before saving. The suggested name is Incident Report.
Capture your first entry
Enter the incident date and location, then choose a type and severity using your organization’s definitions. Describe the observed facts and the actions already taken.
Review and keep it useful
Review the description, photos, and actions as a set. Check any Prevention Recommendation against the known facts and your existing incident process.
